FIRST APPOINTMENT
Welcome to new patients
Please arrive 10-15 minutes early for your appointment for registration and to fill in any necessary forms or questionnaires.
For your first appointment please remember to bring the following:
- Referral letter from GP, family physician or other doctor
- Medicare card, DVA card, Pension Card
- Have your Private Hospital Insurance information with you
- Reports, X-rays, MRI’s, CT scans etc and any other relevant information.

If you need to cancel an appointment – Call (03) 8644 2414
Please call during business hours and allow at least 1 day’s notice. This may allow another patient in need of Dr Love’s care, an appointment.
Special note to patients – We recognise that your time is valuable, and we make every effort to run on time. Occasionally emergencies or patients require a little more time, and these cause scheduling delays beyond our control. We apologise if we keep you waiting.
FOLLOW UP APPOINTMENT
Welcome Back
Please bring the following to your review appointment with Dr Love:
A referral letter – Our staff will inform you if you need a new referral from your specialist or GP.
Imaging Requirements – Please bring all relevant X-rays and/or scans with you to your appointment.
You may be asked to update your:
- Patient registration information – To ensure we have your up to date details
- Medical information – List of medications and known allergies as these may have changed since your last visit.
Please bring to your appointment:
- Medicare card
- Private health insurance details
- CD or films.
